Has anyone received an email from employee relations before regarding an HR concern that has brought to their attention?

I have a colleague who reported her manager for discrimination. Conversations were held . Other people were interviewed to weigh in. Manager received a conversation. Nothing further. This was three years ago and everyone is still here. ("awkward!!")
Yep, someone complained because I asked them a direct question on a call that they didn’t like. I had to explain why I asked the question, then was told It made them uncomfortable and that I couldn’t retaliate in anyway. Nothing happened, it was just an investigation of a complaint.
