Wanted to take medical leave due to mental health and physical illness. But can’t find anyone to contact. The hr system here is a joke. Any tired medical leave before? We need manager approval? I thought we need doctors forms and vendor approval. Why do we need manager approve. He is the reason why I’m doing this. Too afraid to ask manager. He is work miniac he doesn’t care about anything besides work.

All are right answers. Just a little caution which is there will be unofficial and hard to prove harships when you come back if the main basis is mental.
Instead of contacting HR to get the correct answer, I always advise posting HR questions here to internet randos here and picking your favorite answer.
Fill out an absence in myHR.
Send an email to your boss as well stating the reason is correlated for FMLA (you don’t need to give a specific reason).
If they even try to get rid of you, you just won a wrongful termination lawsuit due to the paper trail.
Just fmla or fmla + std?
I don't know about fmla only, but for short term leave, you need a dr to approve it and then file through sedgwick. Sedgwick will provide paperwork for your dr to fill out.
You don't need manager approval for fmla/std.
