How PBMs Hijacked American Healthcare dr-g prescription

When Americans talk about why prescription dr-gs cost so much, we often point fingers at pharmaceutical companies. But behind the scenes, a quiet and far more insidious force drives prices higher — Pharmacy Benefit Managers, or PBMs.

These middlemen were supposed to save us money by negotiating discounts and managing benefits between dr-gmakers, pharmacies, and insurance companies. Instead, they’ve built a cartel-like empire that manipulates prices, restricts access, and drains billions from patients and small pharmacies alike.

Three PBMs — CVS Caremark, Express Scripts, and OptumRx — now control nearly 80 percent of the prescription dr-g market. That’s not competition. That’s consolidation, and it gives them unchecked power to dictate what dr-gs Americans can take and how much we pay for them.

Here’s how the scheme works: PBMs negotiate secret rebates with dr-g manufacturers in exchange for preferred placement on insurance formularies. The larger the rebate, the more likely a dr-g will be covered — even if a cheaper or more effective alternative exists. But those rebates don’t go to patients. Instead, PBMs and insurers often pocket the difference, leaving patients at the pharmacy counter paying inflated copays or list prices.

Independent pharmacies suffer too. PBMs reimburse them below cost, while steering patients to their own mail-order or corporate-owned pharmacies. Many small-town pharmacies — often the only healthcare access point for miles — have closed under this pressure.

It’s legalized extortion wrapped in healthcare jargon.

The result? A system where everyone but the patient profits. Dr-gmakers inflate prices to fund rebates. PBMs boast about “savings” that never reach consumers. Insurers look the other way because they share in the cut. And the average American pays more for prescription dr-gs than anyone else on Earth.

The good news is that lawmakers are finally paying attention. Bipartisan bills in Congress and state legislatures aim to require transparency, ban spread pricing, and force rebates to flow directly to patients. But reform will fail unless regulators confront the core problem: PBMs have become too big, too secretive, and too conflicted to serve the public good.

The United States cannot claim to have a free market in healthcare when three corporations act as gatekeepers to every pill that reaches a patient. We broke up oil trusts and telecom monopolies before. It’s time to do the same for the PBM cartel.

Because healthcare should serve people, not middlemen.

Leaders have started meetings this week to address cutting resources, mostly onshore, before eoy to meet 2026 budget and mgcc staffing commitments.

On a similar topic, there continues to be odd conversations about consolidating into a single Cary building which would only be accomplished by more cuts and offshoring.

The future doesn’t look very good for those stateside.

Analog Devices + Maxim Merger (Closed: August 2021)

CSBBO closures tied to “location strategy,” but they’re still offering OT?

It looks like the CSBBO closures are part of the bank’s location strategy, focusing on consolidating some sites while expanding others.

What stands out is that for the past three days they have been sending messages like this:

If sites are closing, why push overtime right now? It almost seems like they are trying to meet short term goals before more news comes out.

Has anyone else heard which sites are next?


Is it too late in the year for more layoffs?

I'm not sure how it works here, but my old company usually had two major layoffs per year, and it'd all be done by November. Can somebody please tell me how it works here? I've been here for six months, so I'm still trying to figure stuff out. Thanks in advance.

How Many Clinton (Annandale), New Jersey Workers Have Relocated to Houston?

ExxonMobil to close huge N.J. facility as part of consolidation plan

ExxonMobil will close a large research facility in Hunterdon County as part of a plan to consolidate its corporate research operations at its Houston headquarters.

Most of the employees at the Exxon Mobil Research and Engineering division will be offered jobs at the new facility in Texas. The Clinton Township campus, along with a research facility in Ontario, Canada, will close as part of the consolidation, which will be done in phases that will continue until 2028, according to a report by Bloomberg.

“We are incredibly disappointed to learn that ExxonMobil has chosen to close this facility over the next few years,” Clinton Township Mayor Brian Mullay said in a statement. “We will work closely with our partners at Hunterdon County Economic Development & Tourism, the NJ Highlands Council and ExxonMobil to ensure that the developable portion of the property continues to contribute to our area’s economic success and that the ecologically critical areas are preserved to protect our environment.”

In October 2022, ExxonMobil informed the township it had partnered with Prologis do a feasibility study that looked at redeveloping a portion of the property as a warehousing complex. The idea met was met with opposition from the community, according to Clinton Township Council meeting minutes.

In September of last year, Joe Wong, a commercial portfolio manager for ExxonMobil, told the township council that the company was not going to expand beyond the 150 acres that make up its current campus. It last underwent an expansion there in 2017.

The facility employs between 500-600 and pays $3.5 million a year in property taxes, Wong said during a presentation before the council.

Don Carpenter, president of Independent Laboratory Employees’ Union - United Steel Workers Local 09009, said the union the decision impacts over 100 of its members who work at the facility.

“As of right now, none of the unionized workers at the site have been offered relocation or continued employment within the company, however the union will be entering into bargaining with ExxonMobil over the effects of this unfortunate decision,” Carpenter said.

ExxonMobil told NJ Advance Media it is “committed to meeting our obligations under the collective bargaining agreement” and expects “to have further dialogue with them on this announcement.”

Heard that’s the path forward, and it makes sense. Combine operations, leadership. Cut many many many many heads. Sure, it will rough for a while, but it’s all about the immediate savings. No one cares about 3 years from now, only the next qtr and the current gap to solve for. Painful decisions are what random new CEOs are all about.

Also, no more value! Redundant operations. It’s all consumer and business, all customer. We don’t need 10 different marketing teams, just one.


If you are told to relocate

Before taking a package and giving up your right to sue, Jason Hamilton said in his town hall that CSO alone has hired 60 people in Charlotte--a location that has never been on any hub list.

Jobs vs AI vs FICO hit

US based Transunion company tookover Indias largest credit bureau CIBIL 92% stake holder. It's making double digit growth in its India division with largest consumer base close to 2x of US consumer volumes.

TU CIBIL division needs more positions to support its systems including recently acquired Neustar division. It will continue to grow as more digitailization emerging into rural areas. More business more revenues. More revenues More jobs to support.

US division revenues major hit due to low mortgage files with high interest rates fed policies. Less money flow into its division more job cuts.

During Covid TU acquired lot of companies, Its consolidation of tools technologies products and roles are unavoidable for few years.
TU bought UK based callcredit and closed majority of UK employees during last 2 wayves as its revenues are very low. Soon FICO is selling their own scores without paying fee to credit bureaus a major revenue hit to US core markets. World is changing no more a closed Island. Same will continue other countries divisions of TU acquired bureaus.

AI will takeover majority of data science jobs especially engineering and data analytics. AI dont care boundaries, countries and regions.
